When you make your first appointment, you will need to fill out my Intake Packet which includes a:
(1) Client Consent to Treatment Form
(2) Client Personal Information Form
(3) HIPPA Confidentiality Form.  

You have the option to download the Intake Packet from my web site, complete it and bring it with you, or I can have it available for you to complete in my office prior to our session.
